29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:  a)  Jobsite, Route 166, McClellandtown, PA:  On or about June 24, 2015, an employee was operating a Serco 8500 Truck and Trailer Mount Loader from the operators seat that is 12 feet above the ground, and not wearing the provided seatbelt, and fell 7 feet into the truck bed.

29 CFR 1910.333(c)(3): Where protective measures such as guarding, isolating, or insulating were provided, these precautions would not have prevented employees from contacting overhead lines directly with any part of their body or indirectly through conductive materials, tools or equipment:  a) Jobsite, Route 166, McClellandtown, PA:  On or about June 24, 2015, employees were performing right of way clearing operations underneath energized 12kv electric lines and no deneregizing or other protective measures were taken to protect the employees working underneath them.

29 CFR 1910.333(c)(6): Where an employee is required to handle long dimensional conductive objects in areas with exposed live parts, the employer did not institute work practices (such as the of insulation, guarding, and material handling techniques:  a)  Jobsite, Route 166, McClellandtown, PA:  On or about June 24, 2015, an employee was operating a Stihl 131 Pole Pruner that extended about 15 feet, to cut a limb and was positioned approximately 15 feet from an energized 12kv electric line.  The employee was not utilizing any insulating, guarding or other protective measures to minimize the hazards of contact with the electric line.
