29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder: a) First Floor Hallway at Stairway #5 - A drywall hanger was using a Feather Lite, 16 ft., portable extension ladder in a hallway leading to a stairway and the ladder was not secured or provided with a barricade to keep employees away from the base of the ladder, on or about 06/17/2015.

29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause the employee to lose balance and fall: a) First Floor Hallway at Stairway #5 - A drywall hanger was carrying drywall that measured approximately 4 ft., by 5 ft., up a 16 ft., portable extension ladder to hang at the ceiling line and could have lost his balance and fallen off the ladder to a concrete slab below, on or about 06/17/2015.
