29 CFR 1910.305(a)(2)(xi): Cable assemblies and flexible cords and cables were not supported in place at intervals that ensure that they will be protected from physical damage.  Third floor, compressor room, on or about 22 June 2015 and times prior thereto; flexible electrical cable was not installed in a manner to prevent physical damage. Orange Romax Type NM-B AWG 10 Dozier/Claw flexible cable was attached via zip ties to two x four lumber mounted to steel beams. Wire was energized and provides electrical power to the compressor. This condition exposed employees to electrical shock and burn hazards.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed.   Auto shop; on or about 22 June 2015 and times prior; three instances were noted where energized electrical openings were not effectively closed:       a. Tank room  - Electrical cover plate was partially closed.        b. Air handler room 2nd floor - Outlet labeled EF3 had a knockout plug missing.       c. 3rd floor storage area compressor room - Breaker box labeled 26 had a knockout plug missing.   This condition exposed employees to electrical shock and electrical burn hazard.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy.   Storage areas; on or about 22 June 2015 and times prior thereto; two instances were noted where covers were not installed on energized electrical outlet boxes:       a. Air handler room, third floor; an energized electrical outlet box mounted overhead, located 79 inches from the floor did not have a cover plated installed.       b. Second floor storage area; an energized electrical outlet box located below electrical panel labeled AHU-1 did not have a cover plate installed.    This condition exposed employees to an electrical shock and electrical burn hazard.

29 CFR 1910.305(g)(1)(iv)(D): Flexible cords and cables were used where attached to building surfaces.  Office, adjacent to doorway; on or about 22 June 2015 and times prior thereto; a blue 15 foot energized extension cord was used in lieu of permanent wiring. Cord was mounted to the wall with cable clamps. This condition exposed employees to electrical shock and electrical burn hazards.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws.  Tank room, on or about 22 June 2015 and times prior thereto; strain relief was not provided on a cord and plug type emergency light. This condition exposed employees to an electrical shock hazard.      The National Park Service was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.305(g)(2)(iii), which was contained in OSHA inspection number 559578, citation number 1, item number 3 and was affirmed as a final order on 01 October 2012, with respect to a workplace located at the Main Camp Auto Repair section Bay 2.
