29 CFR 1910.253(b)(4)(i): Oxygen cylinders were stored near highly combustible material, especially oil and grease; or near reserve stocks of carbide and acetylene or other fuel-gas cylinders, or near other substance likely to cause or accelerate fire.  Building and Utilities shops; on or about 23 June 2015 and times prior thereto, two instances were noted where compressed oxygen cylinders were unsecure and stored next to flammables:       a. Plumbing shop, a compressed oxygen bottle was stored approximately 4 inches from a Dissolved Acetylene cylinder.       b. Doorway adjacent to tool storage rooms, a compressed oxygen bottle was stored approximately 14 inches from a 55 gallon drum of Triafrost PG Inhibited Propylene Glycol Heat Transfer Fluid.  This condition exposed employees to fire and explosion hazards.

29 CFR 1910.303(g)(2)(ii): In locations where electric equipment was likely to be exposed to physical damage, enclosures or guards were not so arranged and of such strength as to prevent such damage:  Employee parking area; on or about 23 June 2015 and times prior thereto, two instances were noted where electrical conduit was damaged:       a. Pole 5, electrical conduit was cracked exposing electrical wires.       b. Pole 1, electrical conduit was separated from electrical box exposing electrical wires.  This condition exposed employees to an electrical shock and burn hazards.

29 CFR 1910.304(f)(1)(iv): Overcurrent devices for circuits rated 600 volts, nominal, or less, were not readily accessible to each employee or authorized building management personnel.  Plumbing shop, south wall; on or about 22 June 2015 and times prior thereto; the employer did not ensure that the electrical panel was readily accessible in the event of an electrical emergency. Brooms and mops were stored in front of the electrical panel. This condition exposed employees to an electrical shock and burn hazard.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy.  Building and Utilities area; on or about 22 June 2015 and times prior thereto, three instances were noted where electrical covers and/or faceplates were missing:       a. West side B&U building, outdoors, a conduit access box was missing the cover plate exposing wires.       b. Garage bay; a faceplate was missing to an energized electrical outlet.       c. Employee parking lot, pole 2, outdoors, a conduit access box was missing the cover plate exposing wires.  This condition exposed employees to electrical shock and burn hazards.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket).  Specialty shop; on or about 23 June 2015 and times prior thereto; a 12 inch Dewalt Compound Miter Saw, serial number 11357, Model DW708 had damage where the cord and plug attach exposing wires. This condition exposed employees to an electrical shock and burn hazard.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch.  Steam plant shop; on or about 23 June 2015 and times prior thereto; the work rest on a Delta bench grinder Serial number 051943 was not properly adjusted to a maximum of 1/8 of an inch from the abrasive wheel. The gap between the abrasive wheel and work rest measured approximately 1/4 of an inch. This condition exposed employees to a caught in hazard.  The National Park Service was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.215(a)(4), which was contained in OSHA inspection number 559198, citation number 1, item number 1 and was affirmed as a final order on 01 October 2012, with respect to a workplace located at the Toklat B&U Warehouse.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws.  Building and Utilities Shops; on or about 23 June 2015 and times prior thereto; four instances were noted where strain relief was not provided:       a. Southeast wall of B&U shop, outdoors; strain relief was not provided to the plug-in cord. Cord was wrapped with blue tape. Cord is used to plug-in vehicles.        b. B&U Parking lot, pole 3; strain relief was not provided on the cord to the west side pump.       c. Tatlanaka well house, water pump; strain relief was not provide on the well pump. Pump is located outdoors.       d. Steam plant, strain relief was not proved on a heavy duty 25 foot extension cord.  This condition exposed employees to electrical shock and burn hazards.    The National Park Service was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.305(g)(2)(iii), which was contained in OSHA inspection number 559578, citation number 1, item number 3 and was affirmed as a final order on 01 October 2012, with respect to a workplace located at the Main Camp Auto Repair section Bay 2.
