29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch. Sand bay; on or about 22 June 2015 and times prior; a Delta Bench Grinder, Model 23-725; tongue guard was not properly adjusted to a maximum of 1/4 of an inch from the abrasive wheel. The gap between the abrasive wheel and tongue guard measured approximately 5/8 of an inch.

29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose. Tool room 2; on or about 22 June 2015 and times prior; a 120/240 volt electrical breaker panel had dead front panel made of wood and secured with screws to the electrical panel box with screws. This condition exposed employees to electrical shock and burn hazards.

29 CFR 1910.305(g)(1)(iv)(D): Flexible cords and cables were used where attached to building surfaces. Tool rooms; on or about 23 June 2015 and times prior thereto; two instances were noted where energized power taps were substituted for permanent electrical wiring: a. Tool room 1, above the doorway, a Wiremold brand, 15 foot relocatable power tap was stapled to the wall. b. Tool room 2, adjacent to the battery storage area, a Electricord brand, 6 foot relocatable power tap was stapled to the wall. This condition exposed employees to electrical shock and electrical burn hazards.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch. Sand bay; on or about 22 June 2015 and times prior; a Delta Bench Grinder, Model 23-725; tool rest was not properly adjusted to a maximum of one-eighth of an inch from the abrasive wheel. The gap between the abrasive wheel and work rest measured approximately one-quarter of an inch. The National Park Service was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.215(a)(4), which was contained in OSHA inspection number 559198, citation number 1, item number 1 and was affirmed as a final order on 01 October 2012, with respect to a workplace located at the Toklat B&B Shop.
