29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening. Building 19101, Auto shop bay, mezzanine; on or about 23 June 2015 and times prior thereto, the employer did not provide guardrails across the mezzanine storage area, measuring 116 inches above the lower floor. This condition exposed employees to a fall hazard.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). Shop bay; on or about 23 June 2015 and times prior thereto; a 8 inch Black and Decker Circular saw had damaged to the outer jack insulation. This condition exposed employees to an electrical shock and burn hazard.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws. Shop bay; on or about 23 June 2015 and times prior thereto; strain relief was not provided on a Delta Drill Press, serial number 9503. This condition exposed employees to an electrical shock hazard. The National Park Service was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.305(g)(2)(iii), which was contained in OSHA inspection number 559578, citation number 1, item number 3 and was affirmed as a final order on 01 October 2012, with respect to a workplace located at the Main Camp Auto Repair section Bay 2.
