29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. Two instances were noted where employees were observed not wearing head protection: a. Hogan Creek culvert installation; employee was observed not wearing protective headgear while overheard hazard existed. Excavator equipment was being operated during culvert installation. b. Savage River Bridge Project; Flagger was observed not wearing protective headgear while conducting flagging operations. This condition exposed employees to a struck by hazard.
