Employees were working at heights greater than 6 feet without fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  At the jobsite located at 10003 86th Terr., Overland Park, KS; employees using the portable extension ladder to access the roof to perform roofing activities were exposed to fall hazards of up to approximately 17 feet from the roof and ladder to the ground in that the side rails did not extend at least 3 feet above the landing surface of the roof.  The side rails extended approximately 2 feet.
