29 CFR 1926.501(b)(13): 29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:   At the jobsite located at 5011 West 69th Terrace, Prairie Village, Kansas; employees engaged in residential roofing activities were exposed to fall hazards of approximately 9 feet from the eave of the roof to the ground in that fall protection was not utilized.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  At the jobsite located at 5011 West 69th Terrace, Prairie Village, Kansas; employees using the portable extension ladder to access the roof to perform roofing activities were exposed to fall hazards of approximately 9 feet from the roof to the ground in that the side rails did not extend at least 3 feet above the landing surface of the roof.  The side rails extended approximately 1 foot.

29 CFR 1926.102(a)(1): Eye and face protective equipment was not used when machines or operations presented potential eye or face injury:  At the jobsite located at 5011 West 69th Terrace, Prairie Village, Kansas; employees engaged in residential roofing activities using a pneumatic nail gun were exposed to struck by hazards in that eye protection was not utilized.
