29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about June 27, 2015, an employee engaged in trouble shooting activities at the Automatic Tool Changer (ATC 7930) in Building 36 was exposed to amputations hazards in that a machine specific lockout procedure was not utilized while the employee was feeling for alignment.

29 CFR 1910.178(a)(6): The employer did not ensure that all name-plates and markings were in place and maintained in a legible condition: In Building #36, an employee engaged in in material handling activities using a Clark forklift was exposed to struck-by hazards in that the three levers which control the movement of the forks were not labeled.
