29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables shall not be used:  Outside the project fence near an electric power pedestal, the outside insulation was worn down to the insulation on the wires on a flexible cord that provided 110 VAC power to the project.  Employees were exposed to the hazard of contacting electricity.

29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, were not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  Near the west side of the project, lumber with protruding nails were not kept cleared from the work areas and passageways, as nails protruded about 1.5 inches from 2 by 4 lumber that lay next to a flexible cord and in the passageways to the work areas at the site.  Employees were exposed to the hazard of striking against the nails.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:  Near the west side of the project, eye protection was not worn when pounding 16 penny nails into lumber.  Eye protection was not available onsite.  Employees were exposed to the hazard of being struck in the eyes by the 16 penny nails.

29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:  At the project, a 4 by 4 inch square metal electrical box was not used as designed as it lay on the floor instead of being secured on a surface.  The boxes contained a 110 VAC electrical outlet that supplied power to a 110 VAC Makita reciprocating saw, model number JR 3050T, used to cut lumber.  Employees were exposed to the hazards of contacting electricity.
