At the dust collector south location, airlock #2 south, was not locked out during maintenance.   Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  a. A lockout specific operatring procedure was not written for the dust collector and associated equipment, airlock #2 south; energy sources included electrical to the fan/blower, electrical to the airlock, and pneumatic to the blow-down.  Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

29 CFR 1910.147(c)(6)(i)(C): Where lockout was used for energy control, the periodic inspection did not include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected:  a. At least one maintenance employee's periodic review (defined as annual according to the OSHA directive) was last provided over 15 months before the time of the inspection.  b. There was no documentation for at least a second maintenance employee that a periodic review had been provided.  Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.
