29 CFR 1926.34(c): Means of egress was not continually maintained free of all obstructions or impediments to full instant use in the case of fire or other emergency: At the rear exterior of the building on the temporary stairway platform, the means of egress was not free of obstructions, as there were protective helmets, water bottles, and tool boxes stored on the stairs. Employees that used the stairs were exposed to the hazard of falling onto the stairs and equipment.

29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail: At the rear exterior of the building on the temporary stairway platform, there was no handrail on the stairways from the building to the platform. There were four risers on the stairs, and each riser was about 10 inches high. Employees were exposed to the hazard of falling on the stairs and the equipment stored on the stairs.

29 CFR 1926.403(i)(2)(i): Except as required or permitted elsewhere in this subpart, live parts of electric equipment operating at 50 volts or more was not guarded against accidental contact by cabinets or other forms of enclosures, or by any of the following means: In the bathroom in room 107, live parts of electrical equipment were not guarded against accidental contact by enclosures, as a receptacle and single pole switch were hanging by the wires that protruded for electrical boxes in the wall. Masking tape was used to cover the terminals on the receptacle and switch energized at 110 VAC. The outlet provided power through a flexible cord to a 110 VAC chipper used to remove wall tiles, and the switch controlled the light that was illuminated in the room. Employees were exposed to the hazard of contacting electrictiy.

29 CFR 1926.416(a)(1): No employer shall permit an employee to work in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work, unless the employee is protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means: In the bathroom in room 107, the employer permitted employees to work next to a receptacle and single pole switch that were hanging by the wires that protruded for electrical boxes in the wall. 29 CFR 1926.405(j)(2)(i): Receptacles, cord connectors, and attachment plugs were not constructed so that no receptacle or cord connector will accept an attachment plug with a different voltage or current rating than that for which the device is intended: At the rear exterior of the building on the temporary stairway platform, a flexible cord with 220 VAC had a receptacle designed for 110 VAC. Power to the cord was provided by the Commonwealth Utilities Corporation. A 220 VAC Makita right angle grinder was plugged into the cord. The plug on the grinder could be plugged into 110 VAC outlets. Employees that used the flexible cord and grinder to cut ceramic wall and floor tiles were exposed to the hazards of contacting electricity.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables shall not be used: At the rear exterior of the building on the temporary stairway platform, the receptacle on the end of a flexible cord was frayed and pulled away from the outside insulation on the cord. Employees that used the flexible cord to power a Makita right angle grinder were exposed to the hazard of contacting electricity.

Employees that plugged flexible cords into outlets in and near the box were exposed to the hazard of contacting electricity.
