29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated    Front of building:  On or about June 18, 2015, an employee was exposed to a fall hazard, greater than 6 feet to the concrete ground below, while using a JLG Aerial Lift (Model #: 460SJ) with a lanyard that was not attached to the aerial lift.

29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket:     Front of building:  On or about June 18, 2015, an employee was exposed to a fall hazard, greater than 6 feet to the concrete ground below, while standing on the midrail of a JLG Aerial Lift (Model #460SJ) to weld a canopy frame.      South Texas Erectors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.453(b)(2)(iv), which was contained in OSHA inspection number 1006047, citation number 01, item number 002 and was affirmed as a final order on February 26, 2015, with respect to a workplace located at 2915 West Bitters Rd., San Antonio, TX 78209.    South Texas Erectors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.453(b)(2)(iv), which was contained in OSHA inspection number 906929, citation number 02, item number 001 and was affirmed as a final order on July 24, 2013, with respect to a workplace located at 11000 Fischer Road, Von Ormy, TX 78073.    South Texas Erectors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.453(b)(2)(iv), which was contained in OSHA inspection number 546400, citation number 01, item number 002 and was affirmed as a final order on December 04, 2012, with respect to a workplace located at 8703 Bandera, San Antonio, TX 78250.
