29 CFR 1910.141(a)(5): Enclosed workplace(s) were not so constructed, equipped, and maintained to prevent the entrance or harborage of rodents, insects, or other vermin. A continuing and effective extermination program was not instituted when the presence of vermin was detected. Scrap Containment Area - A pigeon infestation was not effectively controlled in the scrap bin and storage area; pigeon droppings were found on the top of stored material and other work surfaces throughout this workplace, on or about June 30, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date, and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.254(b)(4)(iv): Terminals for welding leads were not protected from accidental electrical contact by personnel or by metal objects i.e., vehicles, crane hooks, etc. Protection may be obtained by use of: Dead-front receptacles for plug connections; recessed openings with nonremovable hinged covers; heavy insulating sleeving or taping or other equivalent electrical and mechanical protection. If a welding lead terminal which is intended to be used exclusively for connection to the work is connected to the grounded enclosure, it must be done by a conductor at least two AWG sizes smaller than the grounding conductor and the terminal shall be marked to indicate that it is grounded. a) Main Facility, Welding area: On or about June 30, 2015 the welding lead terminals provided on the Miller Deltaweld 452 Mig welder were not protected against accidental contact. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date, and method of abatement, and states employees and their representatives were informed of this abatement.
