29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: a) 475 Avenel Street, Avenel, NJ 07001. Employees were working on and from Alum-A-Pole pump jack scaffold installing mesh as part the pre-stucco application when the scaffold had not been inspected by a competent person before work started. Employees were exposed to fall hazards in that the safety net was not attached at the base and both end rails were missing. Condition occurred on or about 06/29/15.

Employees were working on and from Alum-A-Pole pump jack scaffold installing new vinyl siding with no fall protection systems in place. Employees were exposed to fall hazards. Condition occurred on or about 06/29/15.
