29 CFR 1910.134(c)(1)(viii): Training of employees in the proper use of respirators, including putting on and removing them, any limitations on their use, and their maintenance.  (a)      Western Maintenance and Construction at 2451 W 82nd Place, Westminster CO 80031:  On or before June 29, 2015, the employer did not provide employees with effective information and training on the proper use and limitations of disposable respirators.  This condition potentially exposed three employees to respiratory hazards during cement sawing and breaking operations without wet methods and while wearing the respirators improperly.

29 CFR 1910.134(d)(1)(iii): The employer shall identify and evaluate the respiratory hazards in the workplace; this evaluation shall include a reasonable estimate of employee exposures to respiratory hazards and an identification of the contaminant's chemical state and physical form. Where the employer cannot identify or reasonably estimate the employee exposure, the employer shall consider the atmosphere to be IDLH.  (a)      Western Maintenance and Construction at 2451 W 82nd Place, Westminster CO 80031:  On or before June 29, 2015, the employer did not conduct employee exposure assessment to determine the proper respiratory protection required for cutting and breaking concrete.  This condition exposed three employees to respiratory hazards.

This condition potentially exposed employees to chemical hazards.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and material safety data sheets.    (a)        Western Maintenance and Construction at 2451 W 82nd Place, Westminster CO 80031:  On or before June 29, 2015, the employer did not provide employees with effective information and training on Respirable Silica Dust in their work area.  This condition potentially exposed three employees to respiratory hazards during cement sawing and breaking operations.
