29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units and the space between the platform and the uprights.  a) On or about June 30, 2015; at the rear of the building or eastside, employees were observed working on a scaffold system that was not fully planked; the employees were exposed to a fall hazard of approximately 25-30 feet to ground level.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.   a) On or about June 30, 2015; at the rear or eastside of the building, employees were observed working on a scaffold system without fall protection in place to prevent them from falling an estimated 25-30 feet to ground level.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge:  a) On or about June 30, 2015; rear of building or eastside, employees were using stairs in which the risers were not guarded exposing employees to falls greater than 6-30 feet to ground level.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:   a) On or about June 30, 2015; rear of the building or eastside, employees were operating electrical equipment, such as but not limited to, portable circular saws that were plugged into a portable extension cord of which the strain relief was damaged, exposing the inner wires.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection.  a) On or about 30, 2015; rear of the building or eastside, employees were operating a portable circular saw that was not plugged into a ground fault circuit interrupter.
