29 CFR 1926.251(e)(8)(iv): Synthetic web slings with broken or worn stitches were not immediately removed from service. Along the west side of the construction building, on or about July 01, 2015, employees were exposed to a struck-by hazard during crane lifting operations of roofing material while using damaged nylon Midco sling.
