29 CFR 1910.178(m)(1): Trucks were driven up to employees standing in front of a bench or other fixed object.    On or about 1 July 2015, and at times prior thereto, employees operating a Hyster stand-up forklift drove it in such a manner as to expose both themselves and others to the hazard of being struck by the lift.  1) On July 1, 2015, an employee drove the lift in reverse while another employee was standing behind the lift next to a pallet rack.  2) On or about June 15, 2015, an employee crushed his foot between the lift and pallet rack when he jumped out of the operator's cab before the lift had stopped moving.

29 CFR 1910.178(o)(2): Load(s) were being handled which exceeded the rated capacity of the industrial truck(s).  On or about 1 July 2015, and at times prior thereto, warehouse associates were using a stand-up lift within the warehouse to lift loads that were in excess of the rated capacity of the vehicle, exposing the employees to struck by and crushing hazards from the lift.

On or about 1 July 2015, and at times prior thereto, the employer allowed a re-locatable power tap to be used in a permanent manner, exposing employees utilizing the power strip to electrical hazards of approximately 120 volts.

29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment.    On or about 1 July 2015, and at times prior thereto, two side-by-side breaker slots were open creating a 2 inch gap in the breaker panel HB, exposing employees accessing the panel to an electrical hazard of approximately 277 volts.

29 CFR 1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening where they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910.    On or about 1 July 2015, and at times prior thereto, four breaker panels located along the back wall of the warehouse had various items stored in front including a ladder, exposing the employee needing to safely access the panels to tripping hazards.

29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed.  On or about 1 July 2015, and at times prior thereto, the emergency exit located along the north east wall had a cart with materials on it that was blocking the route of egress, exposing employees working in the area to a fire hazard.  Floor & Decor was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.37(a)(3), which was contained in OSHA inspection number 705540, citation number 01, item number 001 and was affirmed as a final order on 01/09/2013, with respect to a workplace located at 1980 E. County Line Road, Highlands Ranch, CO 80126.
