On or about 1 July 2015, and at times prior thereto, employees operating a Hyster stand-up forklift drove it in such a manner as to expose both themselves and others to the hazard of being struck by the lift.  1) On July 1, 2015, an employee drove the lift in reverse while another employee was standing behind the lift next to a pallet rack.  2) On or about June 15, 2015, an employee crushed his foot between the lift and pallet rack when he jumped out of the operator's cab before the lift had stopped moving.

On or about 1 July 2015, and at times prior thereto, the emergency exit located along the north east wall had a cart with materials on it that was blocking the route of egress, exposing employees working in the area to a fire hazard.  Floor & Decor was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.37(a)(3), which was contained in OSHA inspection number 705540, citation number 01, item number 001 and was affirmed as a final order on 01/09/2013, with respect to a workplace located at 1980 E. County Line Road, Highlands Ranch, CO 80126.
