OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing for likely to cause death or serious physical harm to employees in that employees were exposed to: Employees operating the burn tables were exposed to pinch points and crushing hazards from the lack of safety steps in the standard work instructions that would instruct the employees on how to put the equipment in safe mode during nozzle change outs during production. Among other methods, one feasible and acceptable means to correct this hazard is to add safety steps to the standard work instructions and retrain employees on the new safety steps. The safety steps should encompass what power sources need to be shut off prior to nozzle change which is part of normal production.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): a) On or before June 30, 2015, the employer did not ensure that the electrical, gas, and pneumatic energy disconnects for the Messer 1 Model Titatn 12/8 S/N #12/LO-12-5656, were operated in a manner that isolated the energy sources prior to authorized employees performing work, in that locks were not applied to each energy source nor were the energy source(s) relieved, disconnected, restrained, or otherwise rendered safe during torch head changes. b.) On or before June 30, 2015, the employer did not ensure that the electrical, gas, and pneumatic energy disconnects for the Messer 2 Model Titatn 10/8LP6 S/N #4575, were operated in a manner that isolated the energy sources prior to authorized employees performing work, in that locks were not applied to each energy source nor were the energy source(s) relieved, disconnected, restrained, or otherwise rendered safe during torch head changes. c.) On or before June 30, 2015, the employer did not ensure that the electrical, gas, and pneumatic energy disconnects for the ITT Model Burny 10 S/N #B10-01-2510, were operated in a manner that isolated the energy sources prior to authorized employees performing work, in that locks were not applied to each energy source nor were the energy source(s) relieved, disconnected, restrained, or otherwise rendered safe during torch head changes.
