29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toe boards: a. On or about July 14, 2015, in the lamination area, employees working on top of the inner hull of the boats, at elevations of four feet or more above ground level, using rollers and brushes to make sure the fiberglass and resin were evenly applied to the surface of the mold, were not protected from fall hazards by a fall protection system. Only the gun runner was using fall protection.

29 CFR 1910.107(g)(2): All spraying areas were not kept as free from the accumulation of deposits of combustible residues as practical, with cleaning conducted daily if necessary: a. On or about July 14, 2015, at the spray area in the small parts lamination department, the employer did not ensure that the carts used to support the molds were kept as free from the accumulation of the combustible residues as practical. The combustible residue accumulated in the carts were cleaned two times a year during the bi-annual shutdown, exposing the employees to fire hazards. b. On or about July 14, 2015, on the floor of spray booths, the employer did not ensure that the accumulation of combustible residues, such as but not limited to gelcoat accumulated on the floor of the booth and filters with overspray were removed as practically as possible. The cardboards with combustible material accumulation were removed every two months and the filters with overspray had not been removed when clogged.
