On or about July 2, 2015, in a 5 foot 5 inch deep excavation near the corner of South Cooper Street, employees were not protected from cave-in from walls by the use of a protective system.    Masci Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1); which was contained in OSHA inspection number 315478214, citation number 1, item number 1 and was affirmed as a final order on 9/21/2011, with respect to a workplace located at 3010 Brown Landing Rd., Palatka, Florida.    Masci Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1); which was contained in OSHA inspection number 315713560, citation number 2, item number 1 and was affirmed as a final order on 12/07/2012, with respect to a workplace located at 100 Scorpion Ct., Satellite Beach, Florida.
