29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Pittsburgh Glass Works, LLC, Packer 2 Line, Snap Run-In Conveyor. On or about June 19, 2015, an employee suffered a severe laceration to the upper, right arm from the snap run-in conveyor on the Packer 2 Line. The conveyer is a series of belts and pulleys with in-going nip points and rotating parts. The machine was not locked out prior to accessing the danger zone of the conveyor when performing servicing and maintenance activities.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by ingoing nip points and/or rotating parts: a) Pittsburgh Glass Works, LLC, Packer 2 Line, Snap Run-In Conveyor. On or about June 19, 2015, an employee suffered a severe laceration to the upper, right arm from the snap run-in conveyor on the Packer 2 Line. The conveyer is a series of belts and pulleys with in-going nip points and rotating parts. The sides of the conveyor were not guarded to prevent employees from the hazards created by the belts and pulleys.
