29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system:    a) Front Street, Philadelphia, PA - The employer failed to install a protective system for the employees working in an excavation approximately 17 feet deep, 80 feet long and 45 feet wide thus exposing the employees to cave-in hazards. This condition was observed on July 6, 2015.

29 CFR 1926.651(i)(3): Sidewalks, pavements, or appurtenant structures had been undermined and a support system or another method was not provided to protect employees from the possible collapse of such structures:    a.) Front Street, Philadelphia, PA - The were employees working in an excavation approximately 45 feet wide by 80 feet long, and 17 feet deep in which the pavement was undermined and a support system was not installed to protect the employees from a possible collapse. This hazard exposed the employees to a struck-by hazard. This condition was observed on or about July 6, 2015.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet or more in depth so as to require no more than 25 feet of lateral travel for employees:    a) Front Street, Philadelphia, PA - The employer did not provide a safe means of egress for the employees working in an excavation approximately 17 feet deep, 80 feet long and 45 feet wide thus exposing the employees to trip, fall and cave-in hazards. This hazard was observed on or about July 6, 2015.

29 CFR 1926.651(j)(2): Employees were not protected from equipment that could pose a hazard by falling or rolling into excavations. Protection by placing and keeping such materials or equipment at least 2 feet from the edge of excavations, or by the use of retaining devices that are sufficient to prevent equipment from falling or rolling into excavations, or by a combination of both if necessary was not provided:    a) Front Street, Philadelphia, PA - The employees were working in an excavation approximately 80 feet long by 45 feet wide and 17 feet deep in which the Komatsu PC 220 LC Excavator was not placed or kept at least 2 feet from the entrance of the excavation nor were any retaining devices used thus exposing the employees to cave-in/struck by hazards. This condition was observed on or about July 6, 2015.

Such protection did not consist of scaling to remove loose material; installation of protective barricades at intervals as necessary on the face to stop and contain falling material; or other means that provide equivalent protection:    a) Front Street, Philadelphia, PA - The employer did not provide adequate protection for the employees from loose rock, soil and debris from the excavation face when working in an excavation approximately 80 feet long by 45 feet wide and 17 feet deep. This hazard exposed the employees to struck-by and cave-in hazards. This condition was observed on or about July 6, 2015.

29 CFR 1926.651(k)(2): Where the competent person finds evidence of a situation that could result in a possible cave-in, exposed employees shall be removed from the hazardous area until the necessary precautions have been taken to ensure their safety.    a) Front Street, Philadelphia, PA - The employees were working in an excavation approximately 80 feet long by 45 feet wide and 17 feet deep in which the competent person did not make a determination of a possible cave-in hazards, safe egress from the excavation, struck-by and/or crushing hazard from an excavator too close to the edge, undermined pavement was not supported, loose rock/debris was not scaled back. This hazard exposed the employees to cave-in/struck by hazards. This condition was observed on or about July 6, 2015.
