29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a) Jobsite: 2nd floor: Employees conducting framing operations around a floor hole were exposed to fall hazards of approximately 8 feet as the hole was not covered or protected with an adequate guardrail system. Observed on or about 6/10/16.
