29 CFR 1926.451(c)(2)(v): The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork:      Levi Kaufman dba Skyline Builders, LP had employees exposed to fall hazards, while working from a supported scaffold lifted by a rough terrain forklift, on a residential construction site in Champaign, Illinois.   The employer did not secure the supported scaffold to the rough terrain forklift by a chain or some other secure method.  Levi Kaufman dba Skyline Builders, LP was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.451(c)(2)(v), which was contained in OSHA inspection number: 894592, citation number 2, item number 2 and was affirmed as a final order on May 06, 2013, with respect to a workplace located at corner of Fellowship Lane and Potomac Avenue, Savoy, IL.
