29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  On December 28, 2016, Levi Kaufman, was issued citations and the employer did not verify, through documents that each citation was abated.  Abatement documentation was not received for the following violations:            a.  Citation 1 Item 1 and,         b.  Citation 1 Item 2 and,          c.  Citation 1 Item 3 and,         d.  Citation 1 Item 4.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  On or about November 9, 2016, Levi Kaufman dba Skyline Builders LP had employees exposed to struck-by hazards in the eyes from the use of pneumatic guns without eye protection, on a construction site in Arcola, Illinois.       Levi Kaufman was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.102(a)(1), which was contained in OSHA inspection number: 894592, citation number 2, item number 1b and was affirmed as a final order on May 6, 2013, with respect to a workplace located at corner of Fellowship Lane and Potomac Avenue, Savoy, IL 61874.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork:  On or about November 9, 2016, Levi Kaufman dba Skyline Builders LP had an employee exposed to 11.5 ft. fall hazards, while working from a supported scaffold lifted by a rough terrain forklift, on a construction site in Arcola, Illinois.   The employer did not secure the supported scaffold to the rough terrain forklift by a chain or some other method.    Levi Kaufman was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.451(c)(2)(v), which was contained in OSHA inspection number: 894592, citation number 2, item number 2 and was affirmed as a final order on May 6, 2013, with respect to a workplace located at corner of Fellowship Lane and Potomac Avenue, Savoy, IL 61874.  Levi Kaufman dba Skyline Builders LP was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.451(c)(2)(v), which was contained in OSHA inspection number: 1075602, citation number 2, item number 1 and was affirmed as a final order on April 8, 2016, with respect to a workplace located at corner of Vahalla Drive and Oakdale Drive, Champaign, IL 61822.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:     On or about November 9, 2016, Levi Kaufman dba Skyline Builders, LP had employees exposed to 11.5 ft. fall hazards, while assessing and exiting the roof via an extension ladder that only extended 1.5 ft. above the upper landing surface of the roof, on a construction site in Arcola, Illinois.      Levi Kaufman was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.1053(b)(1), which was contained in OSHA inspection number: 894592, citation number 1, item number 3b and was affirmed as a final order on May 6, 2013, with respect to a workplace located at corner of Fellowship Lane and Potomac Avenue, Savoy, IL 61874.
