29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system; or, on roofs 50 feet (15.25 m) or less in width, the use of a safety monitoring system alone: (a) Roof above Divine Crown Academy of Cosmetology, Kline Village Plaza, Harrisburg, Pennsylvania - Two employees worked repairing/sealing the roof to remediate roof leaks that had developed. The roof had a height of 12 feet 11 inches above the ground. The employees did not use any fall protection system, exposing them to the hazard of falling to the ground below, on or about July 8, 2015. HEIDLER ROOFING SERVICES, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.501(b)(10), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 621739, CITATION NUMBER 1, ITEM NUMBER 1, AND WAS AFFIRMED AS A FINAL ORDER ON OCTOBER 19, 2012, WITH RESPECT TO A WORKPLACE LOCATED AT 119 NORTH DUKE STREET, LANCASTER, PENNSYLVANIA 17602. Corrected during the inspection. No abatement certification or documentation required.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards: (a) Roof above Divine Crown Academy of Cosmetology, Kline Village Plaza, Harrisburg, Pennsylvania - The two employees who did roofing work, 12 feet 11 inches above the ground on the flat roof, did not use fall protection because these employees did not recognize the need for fall protection, exposing the employees to a hazard of falling to the ground below, on or about July 8, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
