The "Geronimo System" was provided , but not installed at the worksite exposing employees to the hazards of fire, explosion , and/or rig collapse.

The stair did not have railings.  This exposed the employees to the hazard of falling from the stairs.

Two employees were not wearing provided protective eye wear.  This exposed them to the hazard of being struck in the eyes by activity generated debris and well contents.
