29 CFR 1910.268(c): The employer did not provide training in the various precautions and safe practices described in (1) through (3) of this section and did not insure that employees did not engage in the activities to which this section applies until such employees have received proper training in the various precautions and safe practices required by this section: (a) Area of 552 Weaver Mill Road, Rector, PA 15677: On or before July 6, 2015, employees working on the replacement of damaged cable were not given training on First Aid/CPR. In the work area the tree limbs extended through both the cable and the energized 7,200 Volt primary electrical lines exposing employees to electrical shock hazards.

29 CFR 1910.268(q)(1)(i): Employees engaged in pruning, trimming, removing, or clearing trees from lines did not consider all overhead and underground electrical power conductors to be energized with potentially fatal voltages, never to be touched (contacted) either directly or indirectly: (a) Area of 552 Weaver Mill Road, Rector, PA 15677: On or before July 6, 2015, employees were exposed to electrical shock hazards in that approximately 7,200 volts from a primary electrical conductor indirectly energized a tree branch obstructing cable replacement operations. An employee was shocked while cutting the branch.

29 CFR 1910.268(q)(1)(iii): Before any work is performed in proximity to energized conductors, the system operator/owner of the energized conductors was not contacted to ascertain if he knows of any hazards associated with the conductors which may not be readily apparent. 29 CFR 1910.268(q)(2)(i): The employer did not ensure that a close inspection was made by the employee, foreman or supervisor, before working around any tree, to determine whether an electric power conductor passed through the tree, or within any reaching distance. If then, any of these conditions are present an electrical hazard shall be considered to exist unless the system operator/owner has caused the hazard to be removed by de-energizing the lines, or installing protective equipment: (a) Area of 552 Weaver Mill Road, Rector, PA 15677: On or before July 6, 2015, employees were exposed to electrical shock hazards of approximately 7,200 Volts in that after the tree branch shocked an employee the cable lashing operation continued without de-energizing the power lines.
