29 CFR 1910.30(c): The employer did not retrain an employee when the employer had reason to believe that the employee did not have the understanding and skill required by paragraphs (a) and (b) of this section:   a)  Job site, 3639 Massachusetts Avenue, Pittsburgh, PA: On or about August 12, 2024, prior to performing damaged cable replacement activities in an aerial lift, the employer did not ensure that the employee properly understood the fall protection systems and work practices to be followed in order to protect himself. Deficiencies noted during the inspection included, but were not limited to, the employee wearing a harness that was too large for his body and tying off to an area of the lift that was not the engineered tie off point designed by the manufacturer.
