29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact.   a) 3rd Floor Deck - Front: Employee without any eye protection was exposed to eye injury while engaged in framing activities using a pneumatic nail gun , on or about 6/8/15.   b) Ground Level - Front: CSHO observed employee without the use of protective helmet exposed to struck by hazards while climbing an aluminum extension ladder below framing activities, on or about 6/8/15.  Frame Q LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.95(a), which was contained in OSHA inspection number 979064, citation number 01, item number 01 and was affirmed as a final order on 7/22/14,  with respect to a workplace located at 18 Skyline Dr., Englewood Cliffs NJ.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     a) Front: Employee was exposed to a fall hazard of approximately 20 feet while climbing an aluminum extension ladder that did not extend past the working level being accessed, on or about 6/8/15.  Frame Q LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 955187, citation number 01, item number 03 and was affirmed as a final order on 8/12/14,  with respect to a workplace located at 368 Edgewood Ave., Teaneck NJ.
