29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge:  (a): Front Exterior Stairway- Employees were exposed to fall hazards of approximately 9 feet fall while accessing the 1st floor framing deck using a concrete stairway without any handrail, on or about July 11, 2018.  There is no abatement certification or documentation required for this item.

29 CFR 1926.102(a)(1): The employer does not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  Frame Q does not ensure that employees use appropriate eye protection when exposed to eye injury hazards, including but not limited to when using tools such as a circular electric saw, an electric drill, a pneumatic nail gun and a hammer. This violation was most recently observed at 417 3rd Street, Palisades Park, NJ, at instance (a): Exterior Framing- where one employee was not wearing any eye protection while engaged in framing activities using a circular electric saw, on or about July 11, 2018. To abate this violation the employer must ensure that its employees performing framing work are protected from eye injury and that no employee is allowed to be exposed to eye injuries without wearing proper eye protection.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).  Frame Q LLC was previously cited REPEAT for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.95(a), which was contained in OSHA inspection number 1149505, citation number 01, item number 01 and was affirmed as a final order on 03/22/2018, with respect to a workplace located at 119 Abbott Ave, Palisades Park, NJ 07650  Frame Q LLC was previously cited REPEAT for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.95(a), which was contained in OSHA inspection number 1076348, citation number 02, item number 01 and was affirmed as a final order on 03/07/2018, with respect to a workplace located at 462 2nd St Palisades Park, NJ 07650  Frame Q LLC was previously cited REPEAT for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.95(a), which was contained in OSHA inspection number 1042114, citation number 03, item number 01 and was affirmed as a final order on 03/07/2018, with respect to a workplace located at 27 Maple Ave, Mahwah, NJ 07430. 29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels is not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   Frame Q does not ensure that employees use appropriate fall protection while engaged in residential construction activities 6 feet (1.8 m) or more above lower levels. This violation was most recently observed, on or about July 11, 2018, at 417 3rd Street, Palisades Park, NJ, at instance (a): Exterior Framing- where an employee was engaged in wood framing activities and was exposed to fall hazards of approximately 18 feet while standing on top of the framing next to the exterior edge without any method of fall protection and at instance (b): Front Exterior- where employees entering and exiting the structure from the front were exposed to fall hazards of approximately 9 feet while walking across a plank connected to the front stairs. Employees walking across the plank were not protected by any method of fall protection. To abate both instances of this violation the employer must ensure that its employees performing framing work are protected from falls and that no employee is allowed to be exposed to a fall hazard of 6 foot or more without adequate fall protection.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d). 29 CFR 1926.1053(b)(13): The top or top step of a stepladder is used as a step:  Frame Q LLC does not ensure that employees working on stepladders do not stand on the top step. This violation was most recently observed at 417 3rd Street, Palisades Park, NJ, at instance (a): Interior Structure 1st Floor- where an employee was exposed to fall hazards of approximately six feet while engaged in framing work and standing on top of an "A" frame ladder, on or about July 11, 2018. To abate this violation the employer must ensure that its employees who work on step ladders are trained on their use and not allowed to stand on the top step.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
