29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: Parking lot: On or about July 10, 2015, employees were exposed to a fall hazard greater than 6 feet to the dirt ground below while taking vapor readings and standing on top of a 8 foot diameter fuel storage tank without fall protection.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: Parking lot: On or about July 10, 2015, employees were exposed to a fall hazard, of approximately 8 feet, to the dirt ground below, while accessing the tops of fuel storage tanks, while not being provided a ladder or other means.
