Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.254(b)(4)(iv): Terminals for welding leads were not protected from accidental contact by personnel or metal objects:  a)    NAVFAC, Norfolk Naval Shipyard, Building 59, Project Work Center: On or about 06/30/2015, the terminal connections on the Lincoln Ideal Arc TIG-300/300 AC/DC Welders Model #410820 were not protected against accidental electrical contact during welding operations, thereby exposing employees to a possible electrical shock hazard. 29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  a)    NAVFAC; Norfolk Naval Shipyard; Building 59 Project Work Center: On or about 07/13/2015, the flexible cord to the 240 Volts Pittsburgh Machine MP#073495 Lock Former was pulled away from the male attachment plug causing strain on the terminals as a result the internal conductor wires were exposed, thereby exposing employees to a possible electrical shock hazard.   No abatement certification or documentation required.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:  a)    NAVFAC; Norfolk Naval Shipyard; Building 59; Transportation: On or about 07/13/2015, 3 full acetylene cylinders were stowed among multiple oxygen cylinders in a cage and not separated a minimum of 20 feet or by a 5 feet barrier with a fire rating of at least one-half hour.    No abatement certification or documentation required.  Notice 1 Item 6b, 29 CFR 1910.253(b)(4)(iii) as required by 29 CFR 1960.8(b):  The employer failed to separate oxygen and acetylene cylinders in storage. The employer was cited for the same standard and/or same hazard on March 27, 2015, Inspection #1006632.15, April 20, 2015 of final order/settlement and mean of abatement.  Notice 1 Item 3, 29 CFR 1910.253(b)(4)(iii) as required by 29 CFR 1960.8(b): The employer failed to separate oxygen and acetylene cylinders in storage. The employer was cited for the same standard and/or same hazard on May 05, 2015, Inspection #1057074.15, June 5, 2015 of final order/settlement and mean of abatement.
