29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: a) Norfolk Naval Shipyard, Building 59, Portsmouth, VA 23709: On or about August 10, 2017, and times prior thereto, the tongue guards on the abrasive wheels were adjusted to 11/16" and 7/16", exposing employees to potential struck-by injuries. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1904.29(b)(3): Each recordable injury or illness was not entered on the OSHA 300 Log and on an incident report (OSHA Form 301 or equivalent) within seven (7) calendar days of receiving information that a recordable injury or illness had occurred: a) Norfolk Naval Shipyard, Building 59, Portsmouth, VA 23709: On or about August 10, 2017, and times prior thereto, the employer did not enter an injury that occurred on June 26, 2017, that resulted in a fracture and four days away on the OSHA 300 Log and OSHA 301 or equivalent within 7 calendar days. Abatement documentation is required for this item.
