29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold shall be protected by a personal fall arrest system:    a. On or about July 14, 2015, at the above addressed jobsite, an employee installing a window from a ladder jack scaffold approximately twenty-one (21) feet eight (8) inches above the ground was not protected from falls by a personal fall arrest system, thereby exposing the employee to fall hazards.

29 CFR 1926.452(k)(1): Platforms shall not exceed a height of 20 feet (6.1 m).  a. On or about July 14, 2015, at the above addressed jobsite, an employee installing a window from a ladder jack scaffold approximately twenty-one (21) feet eight (8) inches above the ground exceeded twenty feet, thereby exposing the employee to fall hazards.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a. On or about July 14, 2015, at the above addressed jobsite, an employee installing a window was exposed to falls,approximately twenty-two (22) feet above the ground, created by the presence of unprotected sides.    b. On or about July 14, 2015, at the above addressed jobsite, an employee installing a window was exposed to falls,approximately twenty-seven (27) feet above the ground, created by the presence of unprotected sides.
