29 CFR 1910.268(m)(1): Power conductors. Electric power conductors and equipment shall be considered as energized unless the employee can visually determine that they are bonded to one of the grounds listed in paragraph (m)(4) of this section:    (a) Jobsite, 605 Parente Boulevard, Monessen, PA:  On or about 06/29/15 an employee was exposed to electrical shock hazards from coming in contact with energized power conductors.  The employee was attempting to remove a radio transmitter from a utility pole when the employee suffered an electrical shock. The employee did not visually determine that suitable protective grounding was in place that included the following:  A vertical ground wire which was in place and was tested, found safe and was contacted to a power system multi-grounded neutral or the ground neutral of a power secondary system where there are at least three services connected; communications cable sheath or shield and its supporting strand where the sheath or shield is bonded to an underground or buried cable, bonded to a metal piping system or bonded to a power system multi-grounded neutral or grounded neutral of a power secondary system; guys which are bonded to grounds specified which have continuity uninterrupted by an insulator or by arrays of driven ground rods where the resultant resistance to ground will be low enough to eliminate danger to personnel or permit prompt operation of protective devices.
