29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a) Jobsite, on or about July 1, 2015, while installing roof truss and performing other overhead work, employees were allowed to work below and nearby without head protection, exposing employees to struck-by hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) On the roof trusses and on the second floor, on or about July 1, 2015, employees were allowed to work performing framing activity such as, but not limited to installing roof truss without fall protection, exposing employees to fall hazards up to 19 feet 6-inches.

29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail:  a) Jobsite, on or about and at times prior to July 1, 2015, the employer did not ensure stairs with four or more risers were provided with handrails prior to use, exposing employees to trip and fall hazard up to 14 feet.
