29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.      On or about July 14, 2015, at Dollar General Store #3627, in the store aisles and break room areas, employees were exposed to hygiene, trip, slip, and fall hazards when taking breaks and/or handling merchandise without the work area being maintained in an orderly and sanitary condition.      The Dollar General Store was previously cited by Houston North Area Office for a serious violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.22(a)(1), which was contained in OSHA inspection number 728361, citation 1, item number 001, issued on 11/19/2012 and was affirmed as a final order on 12/18/2012, with respect to a workplace located at 405 N. Main St. Madisonville, Texas 77864.      Pursuant to 29 C.F.R. 1903, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all places of employment, passageways, storerooms or service rooms are kept clean and orderly or in a sanitary condition.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:      a) On or about July 14, 2015, at Dollar General Store #3627, at the south entrance door of the receiving/stock room, a portable fire extinguisher was not readily accessible in that it was not mounted and was located behind boxes and rolltainers exposing employees to fire hazards.    b) On or about July 14, 2015, at Dollar General Store #3627, in the rear of the receiving/stock room close to the electrical panel, a portable fire extinguisher was not readily accessible in that it was located behind boxes and rolltainers exposing employees to fire hazards.      The Dollar General Corporation, Store #6125 was previously cited for a serious violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157(c)(1), which was contained in OSHA inspection number 1003589, citation 1, item number 002(a), and was affirmed as a final order on 1/16/15, with respect to a workplace located at 6010 New Hartford Street, Wolcott, NY 14590.       The Dollar General Corporation was previously cited for a repeat other violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157(c)(1), which was contained in OSHA inspection number 989056, citation 1, item number 002, and was affirmed as a final order on 11/25/14, with respect to a workplace located at 2200 Highway 49, Brooklyn, MS 39425.        The Dollar General Corporation, Store #11579 was previously cited for a serious violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157(c)(1), which was contained in OSHA inspection number 907752, citation 1, item number 002, and was affirmed as a final order on 8/15/13, with respect to a workplace located at 1523 East Main Street, El Dorado, AR 71730.
