29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: (a) Slitter Line 7 - Operators and assistant operators were not adequately trained as "Authorized" employees and/or knowledgeable in the recognition of minor servicing procedures that require machine or equipment shutdown and lockout to be performed to prevent injury from operating equipment. On or about July 14, 2015, an employee was injured when reaching into the operating slitter line at the tensioner stand to make a minor adjustment to the material traveling through the unit.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) Slitter Line 7 - On or about July 14, 2015, an employee reached into the operating slitter line at the tensioning stand to adjust the material traveling through the unit by hand without being required to: 1) prepare the machine or equipment for shut down, 2) turn off and shut down the machine or equipment, 3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source(s), 4) affix lockout or tagout devices to each energy isolating device, and 5) verify isolation and deenergization of the machine or equipment have been accomplished prior to starting service and/or maintenance work. This practice exposed employees to the hazard of being caught-in or entangled in running machinery.
