29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    Shop area - On or about July 15, 2015, employees were required to change blades and clean the Smart pallet saw dicmantlers, Baker resaws and, other operating equipment. The employer did not establish an energy control program (lockout/tagout) consisting of energy contrpol procedures, employee training, periodic inspection. Employees were thereby exposed to the hazards associated with moving parts.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    1. Shop area - On or about July 15, 2015, the following equipment were not completely guarded to prevent employees from reaching into rotating moving parts.    a. Smart product dismantler saw guard (cover) was removed and side cover was not closed.    b. Kent resaw unsued portion of blade.    Employees were thereby exposed to laceration/amputation hazards

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  Shop area - On or about July 15, 2015, in the facility, employees were required to operate the Smart Products 1-person Pallet Dismantlers on a daily basis. The horizontal band saw blades on the dismantlers were not guarded. Employees were thereby exposed to lacerations/amputations hazards.

29 CFR 1910.305(b)(1): Conductors entering boxes, cabinets, or fittings.  On or about July 15, 2015, the electrical outlet  to the Smart dismantler saw, was missing a knockout plug, there-by exposing employees to the hazard of contacting electrical parts.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.  Shop area - On or about July 15, 2015, electriccal wires on the Smart dismantler saw was not completly guarded, in that the outer insulation was pulled away from the connector. Employers were thereby exposed to energized conductors.
