29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  a)  Deli Kitchen;  Employees used a corrosive (Pro Power Germicidal Ultra Bleach) to clean pans and an eyewash station was not in the work area.   b)  Beach Glass Kitchen:  Employees used corrosive cleaning chemicals (Swisher Oven Blaster and Swisher Red Alkaline Degreaser) and an eyewash station was not in the work area.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  a)      Laundry Room; Employees voluntarily used a  3M 8210 N95 dust mask respirator to protect themselves from lint dust and they were not given a copy of the information in Appendix D by the employer.

29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or  were not packed or stored to prevent deformation of the facepiece and exhalation valve:  a)       Laundry Room;  3M 8210 N95 dust mask respirators were stored hanging on the wall from hooks by their strap.

29 CFR 1910.1200(e)(1)(i): The written hazard communication program did not include a list of the hazardous chemicals known to be present, using an identity that was referenced on the appropriate material safety data sheet:   a)  The employer did not have an index/ inventory list of hazardous chemicals in the safety data sheet binder for the Garbage Compactor Area.  Employees work with hazardous substances including but not limited to:  Glance HC Glass and Multi Surface Cleaner, Ecolab Laundry Cleaner, Ecolab De-stainer, Chlorinated Bleach, and Ecolab Flexylite Low Alkaline Detergent.   b)  The employer did not have an index/ inventory list of hazardous chemicals in the safety data sheet binder for the Buffet Kitchen. Employees work with hazardous substances including but not limited to:  Bleach, J-512 Sanitizer, Oasis Floor Cleaner, Chlorine Sanitizer, and Swisher Clear Sanitizer.
