29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  Pedro Mendoza had employees exposed to struck-by hazards on the head without head protection, on a residential construction site in Mahomet, Illinois.     Pedro Mendoza was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.100(a), which was contained in OSHA inspection number: 986581, citation number 1, item number 2 and was affirmed as a final order on November 10, 2014, with respect to a workplace located at 1125 Orchard Lake Drive, Aurora, IL.

These fall hazards included:    a. 9 ft., 4 in.  First story roof section and  b. 18 ft.  Second story roof section.  Pedro Mendoza was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(13), which was contained in OSHA inspection number: 986581, citation number 1, item number 4 and was affirmed as a final order on November 10, 2014, with respect to a workplace located at 1125 Orchard Lake Drive, Aurora, IL.
