29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a) 3200 16th St., Zion, IL - In the office, a multi-outlet power strip was being used to supply electrical power to an extension cord, which, in-turn, was supplying electrical power to office equipment including a fax machine. Electrical power strips are not permitted by their listing and labeling to be "daisy chained" to other extension cords exposing employees to the hazard of fires from overloaded electrical circuits.
