29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:        a) On or about November 2, 2016, Federal Equipment & Services, Incorporated did not establish a program consisting of an energy control procedure, employee training, and periodic inspections, to protect employees from unexpected energizing, start up or release of stored energy during service, maintenance, set-up and/or die changes, and inspections of the mechanical power presses.     Federal Equipment & Services, Incorporated was previously cited for a violation of this Occupational Safety and Health Standard, 29 CFR 1910.147(c)(1), which was contained in OSHA inspection number 1078444, citation number 1, item number 1a and was affirmed as a final order on February 5, 2016, with respect to a workplace located at 3200 16th Street, Zion, Illinois 60099.    In accordance with 29CFR 1903.19(c), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

Federal Equipment & Services, Incorporated was previously cited for a violation of this Occupational Safety and Health Standard, 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 1078444, citation number 1, item number 1b and was affirmed as a final order on February 5, 2016, with respect to a workplace located at 3200 16th Street, Zion, Illinois 60099. Federal Equipment & Services, Incorporated was previously cited for a violation of this Occupational Safety and Health Standard, 29 CFR 1910.217(d)(9)(iv), which was contained in OSHA inspection number 1078444, citation number 1, item number 3b and was affirmed as a final order on February 5, 2016, with respect to a workplace located at 3200 16th Street, Zion, Illinois 60099. Federal Equipment & Services, Incorporated was previously cited for a violation of this Occupational Safety and Health Standard, 29 CFR 1910.217(e)(1)(i), which was contained in OSHA inspection number 1078444, citation number 1, item number 4a and was affirmed as a final order on February 5, 2016, with respect to a workplace located at 3200 16th Street, Zion, Illinois 60099.
