29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level: a) On or about 7/17/15, at the site of Amherst, NY. Employee, working from the platforms of a scissor lift, did not chain up the mid chain. Employee was exposed to fall of 12 feet. NO ABATEMENT CERTIFICATION REQUIRED
