29 CFR 1910.157(g)(2): The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment, and at least annually thereafter:  a)  Harbec, Inc., on or about 7/17/15:  The employer has not provided initial and/or annual training to employees on the use and application of fire extinguishers.  * ABATEMENT NOTE:  By this date the employer must either correct the alleged violation or implement a Fire Safety Policy; as outlined in 29 CFR 1910.38(a) and .39(a) which includes the evacuation requirements of 29 CFR 1910.157(b).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a)  Harbec, Inc., on or about 7/17/15:  Dake Johnson horizontal bandsaw was not equipped with an adjustable guard to cover the unused portion of the blade during operation.  The unused blade on the right side measured 6 inches and on the left side measured 9 inches.  b)  Harbec, Inc., on or about 7/17/15:  Tree Journeyman 425 milling machine was not equipped with a guard to prevent contact with the serrated chuck and tooling during operation.  c)  Harbec, Inc., on or about 7/17/15:  Cincinnati Gilbert radial drill press, serial number 1079, was not equipped with a guard to prevent contact with the serrated chuck and tooling during operation.
