29 CFR 1910.95(b)(1): When employees were subjected to sound exceeding those listed in Table G-16, feasible administrative or engineering controls were not utilized:  a) At the recycling room, on or about 7/22/2015: An employee working as the plastic recycling grinder operator was exposed to continuous noise at a sound level of 389.1% of the dose of the permissible exposure limit (PEL). The equivalent dBA for the 389.1% is approximately 99.8 dBA calculated as the 8 hour time weighted sound pressure level. Sampling was conducted for 253 minutes. A zero increment was included for the remaining 227 minutes of the workday.   No administrative or engineering controls were utilized.   Abatement documentation must be submitted for this citation.

29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  a) At the recycling room, on or about 7/22/2015: The employer did not administer a hearing conservation program for employee(s) at the facility who operated the grinder. Monitoring performed by OSHA showed an employee who operated the grinder was exposed to continuous noise at a level of 389.1% of the dose of the permissible exposure limit (PEL). The equivalent dBA for the 389.1% is approximately 99.8 dBA calculated as the 8 hour time weighted sound pressure level. Sampling was conducted for 253 minutes. A zero increment was included for the remaining 227 minutes of the workday.   Abatement documentation must be submitted for this citation.
